We have designed the questions in our Referral form to gather as least amount of personal, sensitive and health information as possible for this stage of the process.

If you want to check out what information you need before submitting a Referral, you can be rest assured that commencing completion of the Referral form and moving through the stages will not send us the provided information until you complete the final acknowledgement at the end.

FAQs

  • The Referral Form is protected across a 256 bit SSL (Secure Socket Layer) connection that uses a SHA256 Certificate.

    The form questions have been designed to ensure we are only collecting what is necessary to consider your referral and more detailed information will be collected if/as necessary, as we progress through the intake process.

    Whilst we are only collecting what is necessary, we also work to ensure the information we gather enables both you and us to make an informed decision about our ability to meet your needs. We’d much rather get it right the first time and avoid any surprises or disappointment! We know how hard it is out there to go through this process, so we hope this helps give you piece of mind that we carefully consider every referral before accepting it.

  • Self-Referral is accepted for all New Adult Clients or Participants, other than DVA or Medicare. DVA and Medicare clients must be referred via your GP.

  • We currently provide services for Adults who are 18 years of age or older, with consideration and flexibility for persons of school-leaving age.

  • In the Method of Service Delivery section of the form, if you select Telehealth or Hybrid Telehealth and Home/Community Visit, you will be lead through a detailed checklist to check your eligibility, that you must be able to meet and accept in order to proceed.

    To determine if you are able to access our Telehealth services, we recommend you check out our Telehealth information page. As well as the eligibility criteria, there’s some more info about risks etc that you need to consider so that you can make an informed decision about using Telehealth.

    Selecting Telehealth or Hybrid Home Visit and Telehealth in our Referral form will guide you through the pre-requisites. Commencing completion of the Referral form and moving through the stages will not send us your information until you complete the final acknowledgement at the end.

  • We anticipate that the form should take no more than 10-20 minutes to complete. However, everyone is different and it may take more or less time, so please ensure to set aside time to complete it when you are least distracted and able to consider all questions carefully, as most are a ‘required’ field.

  • We anticipate to respond to your referral within 7 business days (see our Days & Hours page for more information). However, please bear with us as business demands fluctuate and this time may extend longer than 7 days if we have high demand.

    Once your referral has been reviewed, we will contact you via phone to discuss the details and determine if we are able to proceed.

    If proceeding, we will progress to the Consent to Commence Services Intake Form, which is completed so that we can progress to next steps, which is when we really get down to business!
